Experience the thrill of managing a team and leading the delivery of tailored support services to clients in their own homes. As a seasoned professional, you will oversee the Home Care team to ensure the successful implementation of individualized packages and support services to clients who meet the eligibility criteria for the Home Care Packages Program.

About the Role:

- Oversee the effective delivery of home care services, ensuring that clients receive high-quality support and care.

- Collaborate with the leadership team to develop and implement operational plans aligned with the organizational strategic plan.

- Assess applicants for acceptance onto the program in accordance with HCP / SAH & CDC Guidelines and organizational policies and procedures, and allocate them to appropriate coordinators.

- Ensure that the client group remains consistent with HCP/SAH & CDC target group requirements.

- Promote and represent the organization for the delivery of home care services and community programs.

Key Responsibilities:

  • Lead the Home Care team to deliver high-quality support services to clients.
  • Develop and review care plans in consultation with clients and their families.
  • Build a high-performance team through timely recruitment and effective performance management of staff.
  • Oversee the scheduling of support workers and third-party suppliers to deliver quality care and services.
  • Collaborate with industry groups to enhance the effectiveness and efficiency of service delivery.

Requirements:

- Proven experience in driving high performance and managing community program performance.

- Knowledge of aged care legislation, specifically in home care programs and working towards the Support at Home Program.

- Social, Home & Community Services, Aged Care, Disability, Health Industry or related relevant experience.

Essentials:

  • HR 054: Overseas Criminal Convictions Statutory Declaration.
  • National Police Clearance.
  • Flu Vaccinations ongoing.

Qualifications / Experience:

- Relevant qualification in health or social sciences.

- Experience with budgeting and financial management (desirable).

- Experience in the provision of care services in aged care.

- Experience of delivery of services in the community is essential.
