Community Wellbeing Facilitator

We are seeking an experienced and passionate individual to fill the role of Lifestyle Coordinator in our Residential Aged Care facility. As a Lifestyle Coordinator, you will play a vital role in enhancing the lives of our residents by planning, organizing, and leading social, recreational, and cultural activities that promote independence, quality of life, and overall wellbeing.
You will work closely with our residents, their families, and carers to identify their needs and preferences, and develop programs that cater to these requirements. Your ability to assess and document the effectiveness of these programs will be crucial in ensuring that we provide the best possible care for our residents.
The successful candidate will possess excellent communication and interpersonal skills, with the ability to work independently and as part of a team. You will have a strong understanding of Resident Rights, Aged Care Accreditation Standards, and Outcomes, as well as computer skills in Microsoft Word, Outlook, Excel, PowerPoint, and internet/intranet skills.
