About the Role- To provide patient-focused care and support patients in their own homes, promoting independence and comfort.
- To work collaboratively with multidisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.
- To lead by example, providing guidance and mentorship to junior staff members and contributing to a positive and supportive work environment.
