Job Title: Case Manager Housing First

About the Role:

Housing First is an innovative program that provides young people leaving care with the opportunity to transition to independent living. Our case managers work closely with young people to stabilise their housing, build independent living skills and set them up for a better tomorrow.

We focus on developing the skills and capabilities of young people. Housing First empowers young people to participate in planning and to exercise agency over decisions that impact them, promoting self-management and self-determination.

Key Responsibilities Include:

  • Direct casework with young people to support their transition to independent living.
  • Casework management responsibilities.
  • Developing innovative solutions in a new program.
  • Building partnerships with service providers across the state.
  • Establishing relationships with young people while they are in residential care for up to six months prior to their transition to Housing First housing.
  • Assessment and planning services.

Required Skills and Qualifications:

  • A relevant tertiary qualification in social work, psychology or welfare and experience.
  • A trauma-informed practice lens.
  • A person-centred approach that supports young people to make informed life choices.
  • Tailoring support to meet the needs of each young person.
  • Ability to assist young people to transition from care to independence through safe, stable and secure housing and support.
  • Capacity to provide case management services in complex family circumstances.
  • Sound assessment and planning skills.
