We are seeking a highly skilled Clinical Assessor to join our team. As a Clinical Assessor, you will be responsible for evaluating the needs of elderly clients and determining appropriate care and support services.
Our goal is to provide a cohesive and efficient approach to caring for the elderly, ensuring comprehensive support for our clients.
This role involves conducting home support assessments, developing personalized care plans and coordinating with healthcare providers to ensure clients receive client centred care.
The Responsibilities- Deliver comprehensive and home support assessments using the Integrated Assessment Tool (IAT).
- Collaborate with non-clinical assessors to provide clinical attendance when required.
- Develop and finalise Support Plans that address the holistic needs of clients.
- Ensure accurate and complete recording of assessment information.
- Tertiary qualifications in a health-related discipline are mandatory (e.g., Medical Officer, Registered Nurse, Social Worker, Occupational Therapist, Physiotherapist, or other Health Professional).
- Current AHPRA registration is essential.
- Minimum one year case management experience in assessment and goal orientated support planning in aged, community, health.
- Unrestricted Australian Drivers Licence
- National Police Check
- Access to a motor vehicle with full comprehensive insurance
- Paid mileage and travel time for work-related travel between client visits.
- Hybrid/flexible working arrangements.
- Q Leave entitlement.
- Fully paid training with opportunity to obtain MAC Clinical Assessor/Assessor Delegate certification.
