The Case Manager will work closely with consumers, their families, carers, and advocates to identify goals that shape their personalised Care Plan. You will apply professional judgement to design flexible and tailored solutions that meet each consumer's unique needs.

You should have a passion for working with older people and their families, and be committed to delivering high-quality care and support services.

Key Responsibilities:

  • Assess the needs of consumers and develop individualised care plans
  • Work collaboratively with consumers, their families, carers, and advocates to achieve desired outcomes
  • Maintain accurate and up-to-date records of consumer interactions and care plans
  • Participate in regular team meetings and contribute to quality improvement initiatives
