
Healthcare Service Coordinator

The Outreach Practitioner will be a key member of a multidisciplinary team delivering healthcare services to marginalized and vulnerable adults in Suffolk.
This role involves engaging with hard-to-reach clients, reconnecting those who have fallen out of mainstream services with the care they need.
Key Responsibilities:
- Manage client cases, assessing needs and risks, and delivering outreach services to vulnerable groups.
- Liaise and work with partners serving asylum seekers, refugees, Black and Minority Ethnic groups, ex-offenders, Gypsies and Travellers, homeless individuals, and migrant workers.
- Carry out clinical duties assigned by the Clinical Team Manager, including delivery of care for specific groups of service users.
- Clinical skills and knowledge of NICE guidelines and evidence-based best practice.
- Able to assess, plan, and determine care needs of service users, contributing to multi-disciplinary reviews.
- Professionally registered staff will administer medications, manage medication for service users, and participate in ongoing monitoring of service user medication efficacy and possible side effects.
- Registered Nurse or other professionally registered staff with relevant experience and qualifications.
- Knowledge of mental health legislation and ability to apply it.
- Personal accountability and responsibility for ensuring quality of care standards are met.
