Clinical Aged Care Specialist

We are seeking a highly skilled and compassionate Clinical Aged Care Specialist to join our dedicated team. As a key member of our staff, you will play a vital role in delivering exceptional care and support to older Australians.

The successful candidate will possess a strong understanding of aged care services, with a focus on assessment, planning, and coordination of care. You will be responsible for conducting thorough assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.

Key Responsibilities:
  • Assessment and Planning: Conduct accurate assessments using the IAT to determine client needs and eligibility for home support services.
  • Care Coordination: Review support plans to meet changing client support needs and work effectively both autonomously and collaboratively within a team.
  • Professional Development: Maintain currency of knowledge relating to aged care services and assist with continuous improvement frameworks to evaluate and enhance assessment processes and outcomes.
Requirements:
  • Tertiary Qualification: Tertiary qualification in a health-related discipline directly related to Health, Aged Care or a related specialist area.
  • Registration: Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA).
  • Driver's License: Australian Driver's license and access to a registered, insured vehicle.
  • Experience: Minimum one year demonstrated experience in Australia or Overseas delivering services in Aged Care settings and/or to Aged persons, including those living with dementia.
