
Support Professional – Community Care

We are seeking dedicated support professionals to provide meaningful assistance to individuals with disabilities in a community-focused environment.
Key Responsibilities:- Provide direct care and support to people with varying disabilities, promoting independence and self-advocacy.
- Assist with daily living activities, including personal care, meal preparation, and household tasks, maintaining a clean and safe living space.
- Support individuals in attending medical appointments, accessing community resources, social inclusion, and wellbeing activities, fostering holistic health and wellbeing.
- Monitor and document client progress and care provided, ensuring effective communication with healthcare professionals and team members.
- Collaborate with multidisciplinary teams to ensure comprehensive care, aligning with individual goals and needs.
- NDIS Worker Screening Check or willingness to obtain prior to commencement.
- Availability to work across a full shift pattern, including mornings, evenings, and sleepovers, ensuring flexibility and adaptability.
- Valid unrestricted driver's license, enabling independent travel and transport of clients.
- Experience supporting people with disabilities in community or residential settings, demonstrating expertise and passion for the role.
- Background in related support fields, such as mental health, aged care, or disability support, leveraging existing knowledge and skills.
