Lead Care Specialist
- This role is a critical component of our commitment to delivering the highest quality care to our residents.
The Lead Care Specialist will be responsible for overseeing and resourcing the care needs within the facility, ensuring that all clinical indicators are being met and best-in-class practices are followed. This includes managing assessments, documenting resident information, and submitting AN-ACC requirements.
Key Responsibilities
- Provide expert guidance and leadership to clinical and care service teams across the facility.
- Collaborate with the Clinical Care Manager to lead carer teams in meeting all clinical needs of residents.
- Manage all aspects of assessment, documentation, and submission of AN-ACC requirements.
- Support the effective documentation and full clinical team user support of policies, procedures, and IT system requirements.
- Coordinate assessment, care planning, and review processes.
- Support the admission of new residents for optimal care assessments and initial consultations with residents and families.
- E nsure accurate information is recorded and in accordance with organisational and legislative requirements.
- Input and oversee facility team rosters to ensure optimal skill mix available for delivery of care.
- Manage and document clinical risk and commit to continuous improvements.
To succeed in this role, you will need to have an empathetic approach to care and a commitment to continually improving the quality of clinical care provided. A minimum of 1 year experience as a Care Coordinator or similar leadership position, current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA), and postgraduate qualifications in aged care would be advantageous but not essential.
