At Suncare, we're seeking a talented individual to join our team as an Assessor - Clinical Aged Care. This role involves conducting comprehensive assessments using the Integrated Assessment Tool (IAT) to evaluate and determine the needs of our customers who require home support services.
The successful candidate will develop personalised support plans and assist in connecting relevant service providers to help enable our customers to maintain their independence. As an Assessor – Clinical Aged Care, you'll be working on a casual basis with travel required in the Caboolture region to meet with customers and stakeholders.
You'll need to be available to work a minimum of three days per week. In this role, you'll have the opportunity to collaborate with customers, carers, families, representatives, and other service providers in the aged care planning process.
Key Responsibilities:- Undertaking and reviewing personalised support plans to effectively manage changing customer and carer needs
- Collaborating with customers, carers, families, representatives, and other service providers in the aged care planning process
- Referring for services, assessing complexity, and the risk of vulnerability, and assisting with linking support, reablement, and short-term case management
- Developing emergency care plans and participating in clinical supervision
- Tertiary qualification in a health-related discipline (e.g., Registered Nurse, Physiotherapist, or Social Worker)
- Current unrestricted AHPRA registration or part of, or eligible to be part of, a relevant professional association
- Minimum of one year demonstrated experience directly delivering services in aged care settings and/or to aged persons
- Knowledge or experience in conducting home assessments or risk assessments for vulnerable people, including sensitivity for customers from diverse backgrounds and/or mental health conditions
