Locum Hospital Social Worker

This role involves providing social work support to patients in a hospital setting.

Job Details:
  • Caseload coverage includes Transitional Care Program, Rehab in the Home, and GEM Program.
  • The successful candidate will collaborate with external providers and the My Aged Care system.
  • Liaise with family supports, explore long-term accommodation options, and address other social issues and general patient support and education.
  • Work as part of a multidisciplinary team; join one other senior Social Worker on the team and an allied health team of approximately 10 across these programs.
Responsibilities:
  • Cover a caseload that includes transitional care program, rehab in the home and GEM program.
  • Collaborate with external providers and My Aged Care system.
  • Liaise with family supports, explore long term accommodation options, and address other social issues and patient support and education.
  • Participate in a multidisciplinary team consisting of another senior social worker and an allied health team of 10 professionals.
Requirements:
  • An ASAP start for 8 weeks with possible extension.
  • Prior knowledge of Victorian and NSW Aged Care systems preferred.
  • A driving license is required.
  • 5 days per week, Monday to Friday.
