About the Role

A fulfilling opportunity for dedicated professionals to contribute to the Dementia Support Australia team at HammondCare, delivering high-quality support and care to individuals with dementia.

Key Responsibilities
  • Collaborate with colleagues to enhance the skills and expertise of aged care staff and carers in understanding and supporting individuals with dementia.
  • Implement relationship-based support strategies, modelling and coaching with clients, carers and care staff.
  • Support the implementation of recommendation plans on the ground, utilising modelling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.
  • Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.
Requirements
  • Strong organisational abilities, including effective time management and ability to prioritise in a busy work environment and competing demands.
  • The ability to work both independently and as part of a team.
  • Demonstrated commitment to excellence in customer service principles, including conflict resolution, to ensure the consistent provision of exceptional service to internal and external customers.
  • A current driver's licence and safe driving record.
  • Willingness to travel and work intrastate and interstate for up to five days at a time to support the delivery of DC services.
  • Willingness to work flexible hours at times and as required to meet the assessed needs of clients and carers.
  • Qualification in a relevant field, such as Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health or similar.
