Job Description

We are seeking a qualified Social Support Facilitator to join our team and make a valuable contribution in the lives of older persons from Culturally and Linguistically Diverse (CALD) backgrounds.

You will assist frail and older people to participate in community life and feel socially included through structured, group-based activities that develop, maintain or support independent living and social interaction.

The position is based in Bentleigh East with travel to Glen Waverley and possibly other locations in the Southern Metro region.


Key Responsibilities:
  • Assist frail and older people to participate in community life and feel socially included through structured, group-based activities.
  • Develop and maintain relationships with clients and their families to provide ongoing support and care.
  • Work collaboratively as part of a multi-disciplinary team to achieve positive outcomes for clients.

Required Skills and Qualifications:

- Certificate III in Aged Care or equivalent

- Level 2 First Aid and CPR

- Victorian Drivers licence

- Ability to work in a flexible and professional manner
