This is a fixed-term post for up to 18 months to meet service needs, specifically until March 2027, 30 hours per week. We seek a motivated and enthusiastic practitioner passionate about improving care for people with dementia and their carers to join the Mapping, Education and Carers Service (MECS).

The service covers Gwent, and the post holder must be able to travel across different sites, including care homes and hospital settings. Supervision and professional development opportunities will be provided.

English and/or Welsh speakers are equally encouraged to apply.

Key Responsibilities

Our ideal candidate will:

  • Dementia Care Mapping: Observe and assess people with dementia, supporting care teams effectively.
  • Carers Education: Provide support and education for carers of loved ones with dementia.
  • Staff Training: Design and deliver staff learning and development programs.
