We are seeking a professional to fill the role of Discharge Planner in a permanent part-time capacity.
About the Role:
This is an exciting opportunity for a dedicated individual to join our team as a Discharge Planner. We require someone who is passionate about delivering high-quality patient care and working collaboratively with healthcare professionals to achieve positive outcomes.
Responsibilities will include:
- Assessing the discharge planning needs of patients across multiple wards in liaison with medical, nursing, and allied health providers
- Providing advocacy and support to patients, carers, and their families
- Liaising with family members and external agencies to facilitate future care planning
- Linking patients to local community services and supports
- Attending multi-disciplinary team meetings
Requirements:
To be successful in this role, candidates will require:
- Previous experience as a Social Worker or Registered Nurse
- Knowledge of community resources and services to meet client needs
- Effective interpersonal and communication skills (verbal and written)
- Ability to work independently and within a multidisciplinary team environment
- Commitment to upholding core values
