We are seeking an experienced Care Coordinator to join our team in Bundoora. This is a pivotal role where you will play a crucial part in delivering high-quality care and support to individuals with complex needs.
About the Role:
This is an opportunity for a registered nurse or health professional to make a positive impact on people's lives by coordinating client care needs, including assessment, case management, and developing individualized care plans.
Your Responsibilities:
- Assist the clinical management team with compliance documentation and comprehensive assessments.
- Provide support and supervision of care assistants.
- Implement training programs and deliver training to care staff.
- Coordinate the provision of quality person-centred care according to clients' needs, service/care plans, and available funding.
- Clinical in-home services include care plan assessments, medication management, incontinence care, wound management, chronic disease management, palliative care, and diabetes management.
- Manage client budgets.
- Provide prompt and solution-oriented service to clients.
- Address and manage client complaints/feedback.
Requirements:
- AHPRA registration (preferred).
- Comprehensive understanding of the NDIS framework, policies, and standards.
- Demonstrated understanding of the needs of people living in the community.
- Experience in coordinating programs for older people or people with disabilities in the community and understanding of consumer-directed care.
- Excellent documentation skills.
- Computer literacy and competence in using various apps.
- Excellent communication skills with the ability to liaise and consult with a wide range of clients, health professionals, and team members.
- Own registered vehicle and current driver's license.
- NDIS Worker Clearance and Police Check.
- Right to work in Australia.
