This role presents an exciting opportunity to make a positive impact in the lives of young people. As a Enhanced Care Coordinator, you will provide increased capacity to engage and support complex needs.

Key Responsibilities:
  • Deliver assessment, care planning, referral, advice, and consultation services to young people, their families, and community professionals.
  • Work closely with medical professionals, psychiatrists, intake clinicians, and allied health staff to facilitate coordinated care for moderate and complex mental health disorders.
  • Assess and address the complex needs of young people from diverse communities, including LGBTQIA+, Gender Diverse, or Neurodiverse individuals.
Required Skills and Qualifications:
  • Extensive knowledge and experience working with complex needs of young people.
  • Experience in mental health care, including mental health assessments, risk assessment, and crisis intervention.
  • Advanced skills in crisis management, problem-solving, and negotiation, especially with young people, families, and service providers.
  • Excellent interpersonal and communication skills to build partnerships and effectively liaise with other service providers.
