This role involves managing prospective clients, completing assessments, and creating care plans and HCP budgets. Additionally, it entails working with the scheduler to schedule care plan services, coordinating services including allied health, home maintenance, social support, and regular check-ins with clients for welfare checks.

\

Working collaboratively with PCAs and other support staff and contractors servicing clients, client monitoring and reporting monthly service invoices and statements are key responsibilities of this role.


Required Skills and Qualifications\

To succeed in this position, you will need:

\
  • Experience as a case manager for home/community care.
  • A focus on client care and experience, with a love for the aged population.
  • An understanding of Home Care Packages and their regulations.
  • Good communication skills - both verbal and written.
  • Ability to work collaboratively with a small-medium team.
  • Flexibility and ability to work remotely from home.

Having your own car for when needed to go to client homes or team meetings is also desirable.
