Nurse Care Coordinator

Job Description:

This role offers a rewarding opportunity for a skilled and compassionate nurse to provide holistic, person-centred care to older adults experiencing health challenges and chronic conditions.

The Nurse Care Coordinator will deliver direct clinical nursing care to clients in their homes, conducting thorough assessments, developing individualised care plans and coordinating with healthcare professionals as needed.

The successful candidate will be responsible for providing safe, high-quality nursing practice to clients with complex needs, while ensuring accurate and timely documentation of all care activities.

Key Responsibilities:
  • Providing comprehensive home-based nursing care to clients with specific needs and/or complex needs
  • Conducting regular clinical assessments and developing individualised care plans
  • Collaborating with clients, carers and healthcare professionals to ensure seamless care coordination
  • Maintaining up-to-date and accurate client records
Requirements:
  • Passion for delivering exceptional care to the elderly
  • Current registration with AHPRA following completion of a Bachelor of Nursing or similar qualification
  • Minimum 3 years post-graduate nursing experience with a focus on clinical assessments and care planning
  • Excellent communication and interpersonal skills
