As an Out of Hospital Care Home Care Worker, you will play a vital role in providing support and care to clients in their own homes. You will assist with daily tasks such as cleaning, laundry, transportation, shopping, and preparing meals. You will also provide mobility support, supervise or assist with daily hygiene, and maintain and monitor the client's plan.

To be successful in this role, you will need to have or be working towards a Certificate 3 in Community Services, Individual Support (or approved equivalent) or equivalent relevant work experience. You will also need to have a current First Aid and CPR Certificate, a Working with Children's Check, a National Police Check, and a Drivers License and Car.
