Role Overview

We are seeking a proactive individual to join our Housing and Homelessness Team as a Housing & Homelessness Support Case Worker. The successful candidate will work collaboratively with Coordinators, Case Workers, and Housing Support Staff across our site and programs supporting clients to find and maintain tenancies.

Key Responsibilities
  • Ensure clients obtain access to Home Care Packages (HCP) and NDIS by providing referrals and case management assistance.
  • Conduct initial assessments of new residents upon their entry into the housing program and develop/document support plans as appropriate.
  • Respond to client feedback and complaints in line with organisational policy and procedures.
  • Assist clients to maintain lifestyles of their choice while maximising their health and wellbeing, and addressing issues of social isolation.
  • Facilitate and encourage engagement and maintenance of natural supports.
  • Utilise relationships within sectors to assist access into other services and systems.
  • Work collaboratively with other support workers and teams to ensure a positive team culture.
  • Complete/review client file documentation across program areas to ensure timely and accurate recording of information.

About this Role

This is a Full-Time position based at our Lyndell White House in Coburg, with travel to multiple other metropolitan sites required on an ad-hoc basis.

Requirements

  • A qualification in Social Work, Welfare, Community Services, or equivalent.
  • A current driver's license and willingness to travel to various housing sites around Melbourne.
  • Experience in a support worker role in Intensive Case Management, with specific training and/or experience in AOD, Mental Health, Family Violence, or Homelessness services.
  • Experience working effectively and sensitively with clients, including those with high support needs and challenging behaviours.
  • An understanding of the Housing and Homelessness Service sector, My Aged Care, NDIS, and other relevant service systems.
  • Proven commitment to excellence, accountability, and innovation in service delivery in homelessness and housing support.
  • Strong communication skills and the ability to engage with people from diverse backgrounds.
  • First Aid Certificate.
