About the Role

We are seeking an experienced Clinical Care Coordinator to join our residential care team at Lady McCusker Home in Duncraig. This part-time role involves supervising the daily delivery of care to residents, providing resident advocacy, and coordinating assessments and documentation.

Key Responsibilities
  • Supervise the care team to ensure high-quality care delivery
  • Provide resident advocacy and support
  • Coordinate assessments and documentation to ensure accurate care planning
  • Provide education and coaching to the team to address identified gaps in care
  • Review incidents, investigate and implement corrective actions
  • Drive and monitor ANACC assessments to maximise funding
  • Investigate and manage clinical issues and client complaints
  • Conduct and delegate clinical audits to identify and address clinical gaps
  • Ensure rostered direct care staff are allocated to provide high-quality care
  • Contribute to the performance appraisal process for staff
  • Manage care consumables responsibly and within budget
Requirements
  • Registration with AHPRA for a minimum of 5 years
  • High level of clinical expertise and currency of practice
  • Proven organisational and time management skills
  • Excellent interpersonal communication skills
  • Valid Police Clearance and NDIS Clearance certificate
  • Evidence of flu and covid vaccinations
