Support Coordinator/Recovery Coach Job Description

About us:

We are leaders in mental health service delivery, specialising in working with adults and young people, and in early childhood intervention.

Our mission is to support individuals with psychosocial disabilities to participate fully in the community and enhance their capacity for social and economic participation.

As a Support Coordinator/Recovery Coach, you will work with our team to develop and implement individualized recovery plans that address the unique needs of each client.

You will be responsible for strengthening clients' ability to coordinate and implement supports in their National Disability Insurance Scheme (NDIS) individualised funding packages, supporting their engagement with mainstream, formal and informal services, and developing capacity and resilience in their network.

Key Responsibilities:
  1. Develop and implement individualized recovery plans that address the unique needs of each client.
  2. Strengthen clients' ability to coordinate and implement supports in their NDIS individualised funding packages.
  3. Support clients' engagement with mainstream, formal and informal services.
  4. Develop capacity and resilience in clients' networks.
Requirements:
  • Cert IV in a Community Services related discipline.
  • Experience in a human services environment, particularly experience in understanding service delivery and how it can best deliver for clients with persistent and complex mental health.
  • Ability to meet deadlines in a high demand work environment and to exercise judgment and initiative.
  • Excellent organisational and time management skills.
  • Eligible to apply for NDIS worker screening clearance.
