**Job Overview:**

The NDIS Care Coordinator role is a key position within our organization, responsible for delivering exceptional support services to individuals with disabilities. This role involves coordinating and monitoring the delivery of services outlined in each client's plan, ensuring they are implemented effectively and aligned with the client's individual goals.

**Key Responsibilities:

  • Manage client needs and goals through assessments and care planning.
  • Ensure excellence in client service delivery, including goal planning and onboarding of new clients.
  • Resolve client escalations and complaints in a timely and effective manner.
  • Develop exceptional relationships with both existing and new clients and their families.
  • Liaise with support coordinators to provide feedback on client care.
  • Regularly visit clients to assess well-being and ensure care quality.
  • Provide client-centered support and ensure the delivery of appropriate care.

**Qualifications and Experience:**

We are seeking an individual with a Bachelor's degree in social work, psychology, disability studies, or a related field. The successful candidate will have demonstrated experience in a similar role within the NDIS or disability services sector, as well as strong communication and interpersonal skills. They will also possess the ability to develop and implement individualized support plans.
