Improving Access to Healthcare

This role plays a vital part in enhancing healthcare access for young people from marginalised groups, focusing on those who frequently present to Emergency Departments.


  • Support vulnerable youth to navigate the health care system more effectively.
  • Enhance access to primary and community health services for this cohort.
  • Reduce reliance on Emergency Departments by improving care pathways and outcomes.
  • Develop strong, trust-based relationships with young people facing barriers to care.
  • Identify and address systemic gaps in access to ongoing, coordinated health support.
  • Collaborate with stakeholders to develop frameworks that improve access and equity for vulnerable youth.
  • Contribute to long-term strategies that enhance health outcomes for at-risk populations.

This Clinical Nurse Specialist Grade 2 will work collaboratively with multidisciplinary teams, including General Practitioners, Community Services, Non-Government Organisations, and other service providers to achieve these objectives. The ideal candidate will have experience working with young people from marginalised and at-risk groups, as well as experience working collaboratively within a team.


The successful applicant will be required to have current APHRA registration as a Registered Nurse/Midwife, post-registration qualifications, and at least 3 years' experience working in the clinical area of the specified postgraduate qualifications. Additionally, they will need to demonstrate high-level communication, liaison, negotiation, time management, computer, and presentation skills. Knowledge of Acute service settings and research & evaluation experience, including data collection and data entry, is highly desirable.
