Care Coordinator Job Opportunity

We are seeking a highly skilled and organized individual to fill the role of Care Coordinator. This is an excellent opportunity for someone who wants to make a difference in the lives of clients.

The ideal candidate will have at least 1 year experience with managing client enquiries, client consultations, client onboarding, and a client case load for Home Care Package, NDIS, and private pay clients. They will also have experience managing the complex scheduling requirements for community care workers.

The successful candidate will possess excellent verbal and written communication skills, as well as strong time management, organisational skills & ability to manage competing priorities. They will also have a Certificate III in Individual Support (Ageing, Disability) or equivalent, and current Australian National Police check, Working with Children Check, and Current First Aid & CPR certificate.

Key Responsibilities:

  • Assisting with handling high volume of incoming telephone and internet enquiries
  • Scheduling services for our clients by rostering Care workers dependent on their qualifications and availability
  • Creating master rosters as well as managing daily changes to existing rosters
  • Meeting with clients and assessing their needs
  • This role will handle anything from information requests, feedback, general enquiries to complaints
  • Preparing in-home care assessments and drafting care plans to best meet client requirements
  • Recruiting, training, and managing the performance of the care team
  • Maintaining client and care worker compliance and documentation according to government standards
  • Ensure all documentation is accurately and comprehensively completed in databases and software programs in accordance with organisation policies and procedures and legislative frameworks
  • Providing "on-call" services on rotation after hours, on the weekends, and on public holidays

Preferred Skills:

  • At least 1 year experience as a scheduler, case manager, care coordinator, hospital discharge planner, or NDIS support coordinator
  • Solid knowledge in the Disability and / or Aged Care sector(s)
  • Enrolled Nurse or Registered Nurse will be an advantage
  • The ability to operate efficiently and deliver excellent service when under pressure
  • Intermediate to Advanced Microsoft Office experience (Excel, Word, Outlook, and CMS)
