Job Description

This role is designed to support individuals in addressing key areas that foster their recovery and enable them to participate actively in their communities. The successful applicant will work closely with participants, providing direct, practical assistance to help them regain, keep, and improve essential skills, including self-management abilities.

As a Rehabilitation Worker, you will walk alongside participants, supporting them to identify their hopes, aspirations, and goals, and collaborate on developing tailored recovery plans to help them achieve these objectives. Furthermore, you will be responsible for maintaining accurate records and documentation related to the program, such as participant data and progress updates.
