Job Title: Support Worker

At our organisation, we are seeking a dedicated and compassionate Support Worker to join our team. This is an exciting opportunity for someone who is passionate about providing high-quality care and support to older individuals in their own homes.

The ideal candidate will have a Certificate III in Individual Support (Ageing, Home and Community) or equivalent qualification, and be available to work after hours 3-4 days a week.

About the Role:
  • This is a part-time position, requiring the successful applicant to provide practical household tasks and personal assistance to older people living at home.
  • You will be responsible for ensuring clients receive a flexible and individualised response in accordance with their support plan, and monitoring the ongoing health and well-being of clients and their carers.
  • Maintaining client dignity and respecting confidentiality at all times is essential.
Key Responsibilities:
  1. Assisting clients in their private homes with activities of daily living, including bathing, dressing, grooming, toileting, mobility and light housework.
  2. Providing emotional support and companionship to clients, promoting independence and quality of life.
  3. Ensuring clients receive a flexible and individualised response in accordance with their support plan, taking into account their unique needs and goals.
  4. Monitoring the ongoing health and well-being of clients and their carers, reporting concerns or observations to management as required.
Requirements:
  • Certificate III in Individual Support (Ageing, Home and Community) or equivalent qualification.
  • Available to work after hours 3-4 days a week.
  • Clear police check and working with children check.
  • Own reliable car and valid Victorian driver's license.
