Job Opportunity:

Palliative Care Bereavement Coordinator

We are seeking a skilled professional to coordinate support and bereavement follow-up for family members/carers of patients in the palliative care service.

Key Responsibilities:
  • Coordinate support and bereavement follow-up for family members/carers of patients in the palliative care service.
  • Provide consistent provision of information and psycho-education for bereaved family members/carers of patients in the palliative care service.
  • Educate family members/carers on the availability of bereavement counselling through local resources.
  • Deliver client/patient care within professional, organisational, legal and ethical boundaries, reflecting evidence-based practice and organisational values.
Requirements:
  • Tertiary qualification in Social Work or Psychology with eligibility for membership of a relevant professional association.
  • Advanced clinical knowledge, skills and experience in Social Work/Psychology, with an emphasis on evidence-based practice.
  • Well-developed time management, written and verbal communication, and IT skills, including the ability to use telehealth as a mode of delivery of clinical care.

  • High level of organisational and time management skills.
