Job Opportunity:
Palliative Care Bereavement CoordinatorWe are seeking a skilled professional to coordinate support and bereavement follow-up for family members/carers of patients in the palliative care service.
Key Responsibilities:- Coordinate support and bereavement follow-up for family members/carers of patients in the palliative care service.
- Provide consistent provision of information and psycho-education for bereaved family members/carers of patients in the palliative care service.
- Educate family members/carers on the availability of bereavement counselling through local resources.
- Deliver client/patient care within professional, organisational, legal and ethical boundaries, reflecting evidence-based practice and organisational values.
- Tertiary qualification in Social Work or Psychology with eligibility for membership of a relevant professional association.
- Advanced clinical knowledge, skills and experience in Social Work/Psychology, with an emphasis on evidence-based practice.
- Well-developed time management, written and verbal communication, and IT skills, including the ability to use telehealth as a mode of delivery of clinical care.

- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
- High level of organisational and time management skills.
