A Day in the Life:
  • Lead Care Coordination: Coordinate tailored Home Care Package plans and manage care delivery, budgets, and clinical support to ensure holistic, person-centred care at home.
  • Provide nursing care one day per week, including assessments, treatments, and care planning as part of your role.
About You:
  • MUST HAVE Current Registered Nurse (AHPRA) with hands-on experience and a passion for supporting older adults to live independently.
  • Strong case management skills (ideally in community/aged care), HCP assessment experience, and a goal-oriented, holistic approach.
  • Excellent Communication Skills, valid driver's licence, and willingness to travel locally.
