Ndis Relationship Partner Job

Our ideal candidate is a people-focused professional looking to make a meaningful difference in the lives of NDIS participants. We are seeking an experienced and compassionate individual to join our team as a Ndis Relationship Partner.

Main Responsibilities:
  • Tailor services to meet the unique needs, goals, and preferences of our participants through regular home visits and participant reviews.
  • Conduct thorough assessments, develop, and adapt care plans to ensure the best possible outcomes for our participants.
  • Maintain accurate and up-to-date documentation, working closely with internal teams and external partners to deliver exceptional results.

Key Qualifications:
We are looking for someone who possesses excellent communication and interpersonal skills, is able to work independently, and is passionate about delivering high-quality care.
