About the Role The primary objective of the NDIS Support Partner role is to ensure participants receive professional service and support to meet their care needs.
The role involves working with NDIS Participants to help them achieve their goals while adhering to industry standards.
No two workdays are the same Responsibilities include: Managing an assigned caseload of NDIS participants, ensuring their care and services meet their needs, goals, and preferences Conducting participant reviews and home visits Performing ongoing assessments and reviews of support plans Maintaining all participant-related documentation Collaborating with internal and external stakeholders to provide optimal service Providing operational support for business needs This role is based out of our Tullamarine office with WFH flexibility and requires travel to visit clients as needed.
About You You are a supportive, friendly, and organized individual with excellent time management, leadership, and customer service skills.
You thrive in a dynamic and innovative environment and enjoy being part of a highly skilled team committed to quality care.
You also possess: Experience in a similar role and relevant qualifications Knowledge and experience in disability and/or home and community care A genuine desire to deliver outstanding customer service in all interactions Strong communication and interpersonal skills, with a focus on building meaningful relationships Why Work at Homewell?
