You will work collaboratively with an interdisciplinary team to develop and implement individualized care plans that prioritize patient-centered care and quality of life.
Responsibilities- Conduct comprehensive assessments of patients to diagnose diseases, disorders, and conditions.
- Initiate treatment, including healthcare management and therapeutic interventions, according to statutory and regulatory standards.
- Offer professional guidance to other healthcare professionals and contribute as a key member of the interdisciplinary healthcare teams.
- Collaborate with patients and healthcare professionals to identify trends and patterns impacting clients, families, and communities.
- Develop and implement population-based strategies to enhance health outcomes and participate in policy-making and quality improvement initiatives.
Current registration as a Nurse Practitioner with the relevant nursing authority plus 4 years of recent, relevant clinical experience. Successful completion of the Objective Structured Clinical Examination (OSCE) is required.
