We are seeking a skilled and compassionate healthcare professional to join our team as a Palliative Care Nurse Practitioner.
In this role, you will work with an interdisciplinary team to provide care to patients in their homes, hospices, and acute settings.
Key Responsibilities- Conduct comprehensive assessments of patients, including diagnosing diseases, disorders, and conditions.
- Develop and implement individualized care plans that honor the patient's wishes and enhance their quality of life.
- Collaborate with other healthcare professionals to identify and assess trends and patterns that have implications for patients, families, and communities.
- Current registration as a Nurse Practitioner with the relevant professional association.
- Four years of recent and relevant clinical experience.
- Successful completion of the Objective Structured Clinical Examination (OSCE).
