Continuity Care Specialist

Mater Mothers' Hospital is seeking a skilled and dedicated Registered Midwife to join their team. As a key member of the hospital's Midwifery Group Practice, you will play a vital role in providing continuity of care to women throughout their pregnancy, birth, and postnatal journey.
The successful candidate will be responsible for coordinating, assessing, planning, and implementing care for a caseload of women. This will involve working collaboratively with other healthcare professionals to provide high-quality, patient-centered care.
Key responsibilities include:
- Providing continuity of care to women from antenatal to intrapartum and postnatal periods
- Understanding evidence-based midwifery practice in both antenatal and postnatal periods
- Working within the home environment and partnering with women and multidisciplinary teams
- Supporting the Midwifery Unit Manager in service development and data collection
To be considered for this role, applicants must have:
- Current registration with AHPRA
- Minimum 1-year Postgraduate experience in a Midwifery environment
- Recent experience working in antenatal care, birth suites, and post-natal settings
- Experience working in a continuity of care model is desirable but not essential
- Evidence of continuing educational and professional development
- Demonstrated ability to role model Mater values
