Job Opportunity

We are seeking a dedicated and compassionate Community Care Worker to provide high-quality personal care services to our customers.

The successful candidate will be responsible for delivering care services in accordance with a schedule, which may include personal care needs, social support, transport, and domestic cleaning services.

Key responsibilities will include completing case notes, following instructions from team leaders, maintaining close contact with team members regarding service requests, and communicating customer feedback in a timely manner.

The ideal candidate will have a Certificate III in Aged Care, Community Care, or Disability Care (or equivalent), as well as experience working with home care package recipients within the My Aged Care framework or similar field.

A strong customer focus, empathy for older people and families, and the ability to work autonomously and collaboratively in a multi-disciplinary team environment are essential.

The role requires up-to-date vaccination against seasonal influenza and COVID-19, a current and valid driver's license, and the ability to work various shifts, including afternoons, days, and mornings.

  • Fulfill personal care needs of customers according to a schedule
  • Complete case notes and follow instructions from team leaders
  • Maintain close contact with team members regarding service requests
  • Communicate customer feedback in a timely manner
  • Identify changes in customer needs and risks, and respond accordingly
  • Work safely and adhere to organisational values, culture, Code of Conduct, policies, and procedures
Essential Qualifications and Skills
  • Certificate III in Aged Care, Community Care, or Disability Care (or equivalent)
  • Proven experience working with home care package recipients
  • Basic Microsoft office suite skills
  • Strong customer focus and empathy for older people and families
  • Ability to work autonomously and collaboratively in a team
  • Demonstrated understanding of diversity and ability to work with different cultures
  • Current vaccination against seasonal influenza and COVID-19
  • Current and valid driver's license
