Deliver Exceptional Care to Older Adults

As a Resthaven RN Coordinator Community Services, you will play a vital role in supporting our team of Registered Nurses, Community Coordinators, and Home Support Workers to deliver high-quality care to older adults in their homes.

Your manager will provide guidance and support to help you succeed in this challenging role, which includes an on-call roster. With their backing, you will feel confident in conducting assessments and applying your comprehensive clinical knowledge of aged care programs and services.

Key Responsibilities:

  • Lead a team of Registered Nurses, Community Coordinators, and Home Support Workers to deliver high-quality care to older adults
  • Conduct assessments and apply comprehensive clinical knowledge of aged care programs and services
  • Develop and implement care plans that meet the unique needs of each client
  • Collaborate with other healthcare professionals to ensure seamless care delivery
  • Provide leadership and guidance to your team to ensure they have the skills and knowledge needed to deliver exceptional care

Requirements:

  • Comprehensive clinical knowledge, particularly in age-related issues, and experience working with contemporary practices in an aged care environment
  • An inclusive, collaborative, and supportive leadership style
  • Proficient with computer systems
  • Current AHPRA registration
  • 2023 flu vaccination, and both Covid-19 vaccinations and Covid -19 booster
