Clinical Nurse Specialist Grade 2 Position

We are seeking a highly skilled Clinical Nurse Specialist Grade 2 to join our team at the Illawarra Shoalhaven Local Health District. As a Clinical Nurse Specialist Grade 2, you will be responsible for providing high-quality patient care and contributing to the development of clinical practice in our specialty area.
Key Responsibilities- Provide comprehensive assessments and referrals for patients requiring access to Australian Government funded aged care services
- Develop and implement care plans for patients, including coordination of services and support
- Work collaboratively with multidisciplinary teams to achieve optimal patient outcomes
- Contribute to the development of clinical practice and quality improvement initiatives in the specialty area
- Provide education and support to less experienced staff members
- Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
- Relevant post-registration qualification and at least 3 years' experience working in the clinical area of their specified post-graduate qualification
- High level interpersonal, written and verbal communication skills
- Demonstrated commitment to providing quality service and quality improvement initiatives in workplace practices and procedures
