Job Summary

We are seeking a highly skilled Clinical Nurse Specialist Grade 2 to join our team at the Illawarra Shoalhaven Local Health District. As a Clinical Nurse Specialist Grade 2, you will be responsible for providing high-quality patient care and contributing to the development of clinical practice in our specialty area.

Key Responsibilities
  • Provide comprehensive assessments and referrals for patients requiring access to Australian Government funded aged care services
  • Develop and implement care plans for patients, including coordination of services and support
  • Work collaboratively with multidisciplinary teams to achieve optimal patient outcomes
  • Contribute to the development of clinical practice and quality improvement initiatives in the specialty area
  • Provide education and support to less experienced staff members
Requirements
  • Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Relevant post-registration qualification and at least 3 years' experience working in the clinical area of their specified post-graduate qualification
  • High level interpersonal, written and verbal communication skills
  • Demonstrated commitment to providing quality service and quality improvement initiatives in workplace practices and procedures
