About the Role:

The Clinical Nurse Specialist will be responsible for case managing a portfolio of direct care paediatric clients, ensuring client-centred, individualised care, and positive client outcomes. This role will also involve coordinating and supporting the provision of comprehensive multi-disciplinary care, including assessment, prevention, referral, and health promotion.

Key Responsibilities:

  • To develop and implement individualised care plans for paediatric clients, in collaboration with the multidisciplinary team.
  • To provide high-quality, client-centred care, ensuring the needs of each client are met and exceeded.
  • To coordinate and support the provision of comprehensive multi-disciplinary care, including assessment, prevention, referral, and health promotion.
  • To work collaboratively with the community and clinical nurse team to ensure seamless care delivery.
  • To attract and retain health and support staff for the YNA team, promoting a positive and inclusive work environment.
  • To establish and maintain professional relationships with staff, clients, and their support network.

About You:

The successful applicant will possess:

  • AHPRA registration as a Registered Nurse.
  • Relevant experience in paediatric care, with a focus on community-based care.
  • Exceptional communication and interpersonal skills, with the ability to build strong relationships with clients, staff, and stakeholders.
  • A passion for providing high-quality, client-centred care, with a focus on positive outcomes.
  • Current, unrestricted driver's licence.
  • Current police clearance, WWCC, and other required clearances.
  • Ability to lead and coordinate a team, with prior leadership experience preferred.
