Health Advocacy Specialist

We are seeking a skilled and dedicated clinician to join our team as a Health Coach. In this role, you will provide health coaching for early intervention in chronic disease, with a focus on supporting consumers in developing skills and confidence to manage their health and wellbeing.
Key Responsibilities:- Support consumers in achieving health goals through individualized coaching
- Develop and implement group programs addressing self-management principles such as nutrition, physical activity, medication compliance, emotional health, and connection to support services
- Collaborate with the community to raise awareness of chronic disease self-management
- Current registration with AHPRA as a Registered Nurse with a minimum of 3 years clinical experience
- Post-graduate qualifications in Chronic Disease, Self-Management Support, Health Promotion, or Population Health desirable
- Excellent communication, decision-making, advocacy, and negotiation skills
- Knowledge and experience in facilitating self-management, self-help, or health education/coaching programs
