
Community Care Specialist

We are seeking a dedicated and compassionate Support Worker to provide support and assistance to individuals with a disability.
This role involves delivering person-centered care, enabling participants to access community facilities and participate in community groups. Additionally, you will teach and help with everyday tasks such as meal preparation, daily local errands, and cleaning.
The key responsibilities of this role include:
- Providing support and assistance to individuals with a disability
- Teaching and helping with everyday tasks
- Monitoring and supporting healthcare needs
- Facilitating access to community facilities and participation in community groups
To be successful in this role, you will need:
- Excellent verbal and written communication skills
- Confident computing and smartphone app skills
- Experience in the Community services industry
- A caring approach with a strong work ethic
- Ability to proactively assess risks and solve problems
- Driver's license with full comprehensive car insurance
