Job Title

Aboriginal Care Coordinator - Community Health Specialist.

This is an exciting opportunity to join our team as an Aboriginal Care Coordinator - Community Health Specialist. The successful applicant will provide culturally appropriate care coordination for Aboriginal clients, family members and carers to access timely and appropriate services for cancer prevention and management.

The role aims to improve access, equity and healthcare for Aboriginal people with cancer, promoting patient-centred care and a holistic approach to health and wellbeing.

The Aboriginal Care Coordinator - Community Health Specialist will ensure care coordination for each person is informed by factors such as connection to land, gender, kinship, inter-generational trauma, family ties, language barriers and socio-economic issues.

  • Provide culturally appropriate care coordination for Aboriginal clients, family members and carers to access timely and appropriate services.
  • Improve access, equity and healthcare for Aboriginal people with cancer.
  • Care coordination for each person informed by factors such as connection to land, gender, kinship, inter-generational trauma, family ties, language barriers and socio-economic issues.
Key Responsibilities

The key responsibilities of the Aboriginal Care Coordinator - Community Health Specialist include:

  • Supporting and guiding Aboriginal clients, family members and carers to access timely and appropriate services.
  • Improving access, equity and healthcare for Aboriginal people with cancer.
  • Care coordination for each person informed by factors such as connection to land, gender, kinship, inter-generational trauma, family ties, language barriers and socio-economic issues.
Requirements

To be successful in this role, you will need to have:

  • Hold qualifications in Aboriginal Primary Health Care or other health-related discipline and/or equivalent experience.
  • Active involvement and 'lived experience' in Aboriginal communities, with a deep understanding and commitment to improving the health issues impacting Aboriginal communities.
  • Understanding of cancer care provision and ability to guide Aboriginal people through the system in a culturally safe way.
  • Healthcare experience and sound leadership skills to facilitate effective and culturally appropriate education and training to staff.
  • Ability to develop and maintain strong stakeholder relationships and confidence in negotiating with clinicians as part of the multidisciplinary team to advocate for best patient care.
Selection Criteria

The selection criteria for this role includes but is not limited to:

  • Applicants must be of Aboriginal descent through parentage, identification as being Aboriginal and being accepted in the community as such.
  • Exemption claimed under Section 14 of the Anti-Discrimination Act 1977.
