Responsibilities
  1. Evaluate, design, and implement individualized patient plans of care.
  2. Participate in local community service initiatives to promote health and wellness.
  3. Mentor and support new clinic staff to enhance their skills and knowledge.
  4. Complete timely and accurate patient documentation, including initial evaluations, progress notes, discharge summaries, daily SOAP notes, functional status reports, functional capacity evaluations, and insurance/plan of care forms as necessary.
  5. Provide community outreach and engagement activities, such as hosting booths, presenting in-services, and conducting complimentary screenings.
  6. Establish and develop professional relationships with referral sources, including physicians, PAs, ARNPs, chiros, case managers, and other healthcare professionals.
Requirements
  • Degree from an accredited Physical Therapy program.
  • 1-2 years of experience in physical therapy, preferably in a similar setting.
  • Active clinician license or compact privileges in the state where services will be provided prior to beginning assignments.
  • Current CPR certification.
  • Ability to read, write, and speak English proficiently.
