Clinical Care Specialist

We are seeking a passionate Registered Nurse to deliver exceptional care and medical support in the community.
Key Responsibilities:- Undertake general nursing duties, including client assessments, medication management, and care planning/plan review.
- Engage with stakeholders to ensure appropriate care for clients.
- Collaborate with the clinical team to achieve objectives and promote a continuous improvement program.
- Act as a role model and mentor for colleagues.
- Develop and review protocols for relevant clinical areas.
- Provide clinical training and education to staff.
- Attend to clients to deliver specific clinical care/interventions.
- Represent the clinical team at client meetings and review incident reports.
- AHPRA registered nurse with no restrictions.
- Postgraduate qualifications in a relevant field (advantageous).
- Minimum 2 years' experience in a clinical role.
- Experience in community/nursing settings preferred.
- Strong focus on person-centered care and quality outcomes.
- Excellent communication skills and stakeholder management.
