Job Description

We are seeking a skilled and motivated individual to join our team as a Habit Coach. In this role, you will have the opportunity to make a real difference in people's lives by supporting them in developing life-changing habits.

You will work closely with our highly qualified Allied Health professionals to carry out duties that complement the therapy program of participants. Your role will play a vital part in enabling participants to develop greater independence and work towards reaching their full potential.

The Role
  • Work under the guidance of our experienced professionals to carry out face-to-face and non-face-to-face tasks that bring life-changing experiences to participants.
  • Carry out duties that promote participant independence and empower them to reach their goals.
  • Develop strong relationships with participants and their families to provide ongoing support and guidance.
Key Responsibilities
  • Carry out assessments and interventions to identify and address participant needs.
  • Develop and implement personalized plans to support participant goals and outcomes.
  • Collaborate with multidisciplinary teams to ensure seamless delivery of services.
Requirements
  • A relevant qualification in health or social sciences.
  • Experience working with individuals with disabilities or complex needs.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
