Your expertise will help ensure that our clients receive the best possible care and support to maintain their independence and quality of life.Responsibilities:
  • Conduct initial assessments to understand client needs and preferences
  • Develop and implement personalized care plans to meet client goals
  • Provide ongoing support and care to clients in their own homes
  • Work collaboratively with other healthcare professionals to ensure comprehensive care
Requirements:
  • Certificate III Aged Care or Individual Support qualification
  • Current Australian Driver's License (Green Provisional minimum)
  • Roadworthy and insured vehicle for work use
  • Experience working with technology, including smartphones and laptops
