Key Responsibilities- Develop and implement individualized care plans that prioritize the needs and goals of each child.
- Collaborate with a multidisciplinary team of internal and external stakeholders to ensure comprehensive support for children and their families.
- Provide crisis intervention and support to children exhibiting high-risk behaviors or trauma-impacted backgrounds.
- Document and maintain accurate records of child progress and care plans using DFFH Service systems CRIS.
- Tertiary qualifications in social work, psychology, youth work, or a related field.
- Experience in a case management setting, preferably in a residential care environment.
- Strong leadership and communication skills, with the ability to work effectively with diverse stakeholders.
