We are seeking a highly skilled and dedicated SIL Care Coordinator to join our team at Direct Care Assistance. As a key member of our organization, you will play a vital role in delivering exceptional support to our NDIS participants in Supported Independent Living (SIL) settings.

Key Responsibilities
  • Client Support and Care Planning
    • Develop and implement personalized care plans in collaboration with participants, their support networks, and care teams.
    • Conduct regular meetings with care teams to review and update care plans.
  • Communication and Collaboration
    • Efficiently communicate with participants' support teams, including support coordinators, guardians, advocates, and family members.
    • Collaborate with DCA operations manager and other stakeholders to ensure seamless support delivery.
  • Care Coordination and Quality Assurance
    • Monitor and ensure the delivery of high-quality care by reviewing participant reports, care plans, and addressing any issues promptly.
    • Confirm that support workers fully understand and can meet participants' needs in line with DCA's values.
  • Administrative and Reporting Tasks
    • Maintain accurate participant and team records in DCA's record system.
    • Handle the onboarding process for new clients, including initial assessments and compliance documentation.
Requirements
  • Qualifications and Experience
    • Must have qualifications in disability services.
    • Proven experience in the disability sector, specifically within NDIS services and SIL settings.
  • Skills and Abilities
    • Strong communication and team engagement abilities.
    • Excellent organizational skills and meticulous attention to detail.
    • Ability to work both independently and collaboratively.
    • Commitment to upholding the rights and dignity of individuals with disabilities.
  • Certifications and Licenses
    • Certificate 3 or 4 in Disability.
    • Driver's License.
    • NDIS Screening check.
    • Current First aid certificate.
    • Current CPR.
    • Police Check.
