Social Services Coordinator

We are seeking a skilled professional to provide assertive outreach services to individuals and families who are homeless or at risk of becoming homeless.
About the Position- To develop early intervention strategies to engage participants and achieve both program and individual goals.
- Conduct initial assessments to ensure eligibility for our programs and ongoing assessments in line with work instructions.
- Use tools like the Outcome Star to track participant progress and barriers to achieving goals.
- Maintain accurate electronic records of participants' personal information.
- Negotiate community partnerships via networking and referrals.
- A degree-level qualification in Social Work, Human Services or Behavioural Sciences, or a Certificate level 4 or higher qualification in the community sector.
- Excellent written and verbal communication skills, strong analytical skills, and a current driver's license.
- Holding or being eligible for a Disability Worker Screening and Working with Children Blue Card is essential.
